Letter Of Agreement

I have read and understand my obligations and responsibilities as a member of the North Pulaski High School Band.  I agree to follow all rules and do my part to make the NPHS Band the best it possibly can be.  I have received a CALENDAR and a copy of the GRADING SYSTEM, DEMERIT SYSTEM and LETTERING SYSTEM for the current school year.

This form is due in the band office by the first day of school. Failure to return this form and instrument checkout form (if applicable) will result in the student being removed from performances.
